The Little Sisters of the Assumption Family Health
Service’s Certified Home Health Agency provides skilled
nursing, medical social work, physical and occupational therapy
and home
health aide services to adults and children residing in East Harlem.
We serve residents from 96th Street north to 132nd, and from
5th
Avenue to the East River. Patients are of all ages, but more than
half are children and pregnant and post partum women. Home visits
are family centered and we link families to other programs within
the Agency as needed. Many of the families referred to the nursing
program are the very poor and the marginalized who, “fall
between the cracks” of the larger institutions.
The Certified Home Health Agency (CHHA) is staffed
by five community health nurses and an administrator and nursing
supervisor. It includes three special programs that address
acute
needs of the East Harlem community: The Maternity
Outreach Program, the Asthma Prevention/Intervention
Program, and the Diabetes Program.
